Meaningful Moments ®
5 Cornerstones

Our exclusive Meaningful Moments® program, founded upon innovation and education, is specifically designed to help meet the needs of those residents with memory loss by helping them to bond with caregivers through an exploratory and scientifically-proven approach to relationship building. We begin by discovering the life story of each individual’s likes, dislikes, and cherished memories. By adding details and preferences from each resident’s life story into their day, we can build comfort and a sense of meaning for each individual resident. The result is a familiar and family-like atmosphere for your loved one.

Meaningful Moments focuses on engaging our residents where they are, allowing them to function safely within their environment at their highest possible potential. We achieve Meaningful Moments with all of our residents through the five cornerstones of our program:

Purposeful Programming

A Meaningful Moment is created when one of our team members provides a unique and purposeful interaction for a resident based on their individual life story. This can happen many times throughout the course of a resident’s day.

 

Meaningful Moments include both scheduled and unscheduled programs with an emphasis on supporting our resident’s holistic wellbeing. Meaningful Moments are meant to provide residents with Alzheimer’s and related dementias the opportunity to function at their highest potential, in a safe, caring environment.

Signature Dining

Interaction with our residents happens all day – so why should engagement stop at mealtimes?

 

Our Signature Dining element of Meaningful Moments embraces healthy meals and nutritious snacks to promote health and hydration throughout the day. With three meals and three snack times over the course of a day – we offer six daily opportunities – or 42 mealtime interactions per week to engage with residents.

 

Through Signature Dining our teams create Meaningful Moments, maximize independence, and promote socialization all in an inviting, safe environment.

Training, Education, & Retention

As one of the leading providers in Alzheimer’s and dementia care, we are committed to continual education and growth opportunities for all of our team members.

 

With our strong onboarding processes and extensive Meaningful Moments training program, we are able to successfully keep both new and existing team members informed of the latest care techniques and best practices for safety and engagement.

 

Extending our educations resources to resident families allows us to better partner with them in their loved one’s care. Family members are an integral component of Meaningful Moments and are invited to join us for family orientations and ongoing educational support groups.

Family Partnership

Entrusting a loved one’s care to someone can be a difficult decision for families to make.

 

Often a family member has been the sole caregiver for their loved one. When the transition to a memory care community comes, family caregivers and loved ones can experience feelings of guilt, loss, and helplessness.

 

As part of our Meaningful Moments program, we coach families through this transition and provide the resources to cope with this change. For many family caregivers, they have only been spending time “caring” for their loved one and providing support with Activities of Daily Living. After transitioning their loved one to our community, they have found that they are able to engage in meaningful connection again, and leave the holistic wellbeing of their loved one in our hands.

 

Our family partnerships allow us to give families a stronger and more positive relationship with their loved ones. Through care conferences and frequent communication via the Sagely app, we encourage families to stay as involved as possible as we all partner together in their loved one’s healthcare.

Dementia Live®
for family & professional caregivers

Dementia Live® is a high impact, dementia simulation experience that immerses participants into life with dementia, resulting in a deeper understanding of what it’s like to live with cognitive impairment and sensory change.

This is a 30 minute program with certified Dementia Live® trainers, offering a unique inside-out understanding of dementia and aging. Participants will gain a heightened awareness of the challenges faced by those living with dementia. Learn valuable tips and tools to improve communication and care.
